Steve Gold on Scaling Without Outgrowing Your EHR
Steve Gold founded Refresh Mental Health in 2018 with a straightforward mission: make quality behavioral health care more accessible. He didn’t start with a blueprint for a national brand; he started with a handful of clinical partners in Florida and Alabama, a parent company model, and a belief that good operations could make great clinical care possible.
Running the business quickly become complicated as he introduced new clinical service lines and grew the staff. Refresh dealt with different billing requirements in each of the markets they operated in. The challenge of serving clients across outpatient therapy, medication management, eating disorders, and intensive programs, all at once, under one operational umbrella, was significant. And underneath all of it, a fundamental question that every growing practice eventually faces: can our EHR keep up?
We pulled from Steve’s account of the Refresh story, in his own words, because his experience cuts closer to home than you might expect.
“We Needed a Partner, Not Just a Platform”
When Refresh started acquiring local clinical partners, each one arrived with its own systems, habits, and workflows. Standardizing across them, without flattening what made each practice work, was the actual problem Steve was trying to solve.
“We were rapidly growing at Refresh, and to effectively sustain that rate of expansion, we needed an EHR partner that was scalable, cost-effective, and flexible,” he says. “Our diverse behavioral health services across multiple states required comprehensive EHR capabilities and adaptability, which were both demonstrated by PIMSY.”
The word he comes back to is flexible. Not feature-rich. Not powerful. Flexible.
That distinction matters. A rigid EHR can have every feature on the checklist and still fall apart the moment your organization stops fitting the mold it was built for. Adding a prescriber changes your workflows. Adding an IOP track changes your documentation. Adding a second location changes your reporting needs. The EHR that worked at 3 clinicians often doesn’t work at 12, much less at 50 clinicians.
Why PIMSY
Refresh didn’t choose PIMSY without a track record to point to. They’d already watched it in action at Comprehensive MedPsych Systems, a 146-person behavioral health practice operating across two states and multiple time zones. CMPS had gone live with PIMSY in record time, under real operational pressure.
“Something else that set PIMSY apart from the competition was their dedication to our success early on,” Steve says.
Practice owners often evaluate EHRs like any other software purchase: features, price, implementation timeline. Steve’s framing is different; what he found wasn’t a vendor. It was a team that understood the mission, improving access to mental health care, and built around that.
“PIMSY clearly understood our mission at Refresh: improving access to mental health care while maintaining operational efficiency across a rapidly growing network.”
What Actually Changed
Once Refresh was running on PIMSY, the operational picture shifted. Custom reporting gave leadership visibility across the organization without the manual overhead. Billing automation handled different payer structures, contract types, and service lines without requiring a workaround each time something new was added. When Refresh layered in IOP or psychiatric medication management, PIMSY already had workflows for it. The EPCS and PDMP integrations kept prescribing compliant, and PIMSY’s API connected to the analytics and payroll systems the finance team depended on.
“The PIMSY platform was able to fully support quickly changing needs with custom reporting, automation tools, and API integrations,” Steve says. “This, and their proactive approach to interoperability, billing automation, and regulatory compliance, set them apart from other EHR providers.”
The part that gets missed when you’re evaluating EHRs early on: you’re not buying what you need today. You’re buying whether the platform can handle what you’ll need in two years.
What Every Growing Practice Owner Should Hear
The growing pains Steve describes at Refresh aren’t unique to large organizations. They show up the moment you add a second clinician or a prescriber, and certainly if you add a second location or new clinical services: each layer of complexity asks the same question of your EHR: can it keep up?
The practices that scale well aren’t the ones scrambling to find a new system once they’ve outgrown the old one; they’re the ones that made the right call early on.
Refresh grew into an organization strong enough that a major health system wanted to acquire it. That outcome started with the same operational decisions every practice owner faces now: what to offer, how to coordinate a growing team, and whether the technology underneath it all can carry the weight.
The answer to that last question changes everything.
