Every accepted superbill needs the same core building blocks. The practice header carries the practice name, address, phone, email, and website. The provider block carries the rendering provider name, credentials, NPI, tax ID or EIN, and license number. Payers use the NPI and tax ID to identify who delivered and billed for the service, so this tool validates that the NPI is ten digits and flags an entry that does not pass the standard NPI check digit.
The client block captures the patient name, date of birth, address, and contact details, along with the insurance company, member ID, group number, and subscriber relationship. When the subscriber is not the client, for example a child on a parent’s plan, you can record the subscriber name separately. Missing insurance details are one of the most common reasons a superbill cannot be filed, so the tool warns you when the payer name and member ID are both blank.
Diagnoses and services are where coding accuracy matters most. You can add up to four ICD-10 diagnosis codes, each labeled with a pointer from A to D. Each service line then references the diagnosis it supports through that pointer. The tool checks that diagnosis codes follow the ICD-10 format of a letter followed by two digits with an optional decimal, and that each service code is a valid five-digit CPT or an alphanumeric HCPCS code.
For every service line you record the date of service, the CPT or HCPCS code, a plain description, the diagnosis pointer, units, the fee charged, the amount paid, any adjustment, and a place of service code. The tool calculates the line balance as fee charged minus amount paid minus adjustment, then totals the charges, payments, and adjustments across all lines to show the overall balance due. Place of service codes let you distinguish in-office visits from the two telehealth codes, which can affect how a payer processes the claim.
The payment summary records the payment date, method, and any notes, along with a provider attestation line.
